This is one of the trickiest parts of treating patients with ALS. There is a frontal type dementia that is part of the disease and as several authors have pointed out, it is this general lack of insight and judgment that contributes to the willingness to "try anything" not just the desperation of being faced with a horrifying disease. It is essential to keep this fact in mind when discussing such things as diet options, tube feeding etc.
