[Dysphagia] Wegener's granulomatosis

Wegener's is only one of over 300 conditions that cuae
swallowing impairment. Not all dysphaigais are
primarlity neurologic. Many are caused by alerations
in the muscle itself e.g. polymyositis etc. Please
see:
Loehrl TA; Smith TL Inflammatory and granulomatous
lesions of the larynx and pharynx. Am J Med 111:113-7.
2001.
A number of inflammatory and granulomatous lesions can
involve the larynx and pharynx. They may be difficult
to diagnose because of the range of symptoms. This
article reviews the following conditions:
supraesophageal complications of reflux disease,
relapsing polychondritis, Wegener granulomatosis,
sarcoidosis, tuberculous laryngitis, Teflon granuloma,
amyloidosis, rheumatoid arthritis, and systemic lupus
erythematosus. The purpose is to provide a brief
review of each disease and its manifestations,
symptoms, diagnosis, and treatment.

Mozaffar T; Lopate G; Pestronk A . Clinical correlates
of granulomas in muscle. J Neurol 245:519-24.1998.
Case report of ten patients with granulomas in their
muscle biopsy specimens. Of these, eight had myopathic
changes. Seven had dysphagia as a major functional
difficulty . None had elevated levels of serum
creatine kinase (CK). Granulomatous myopathy, in the
presence or absence of sarcoidosis, is commonly
associated with dysphagia (87%) and a normal serum CK.
Granulomatous myopathy without systemic sarcoidosis
was associated with milder, but more predominantly
distal weakness.
