[Dysphagia] Free Water Survey

For those of you with a long memory, I started a survey of facilities using 
free water back in October 2004. I have compiled a start to this survey. 
Please e-mail me off list if you would like a copy in PDF e-mailed to you. 
I'm hoping this is just the beginning of this data file and others who are 
using the protocol will send their results to me. I would like this to 
become an ongoing reference for those wanting to implement free water in 
their facilities. You are encouraged to send me updates as you progress in 
using the protocol. If you know of anyone using the protocol, please let me 
know how to contact them for inclusion in the data. Thanks for your patience.
