[Dysphagia] physician view of dysphagia tx



Pam has kindly supplied the URL for the article you refer to. Most of the geriatricians I know have serious reservations about the use of such things as thickeners and many, if not most, of the recommendations made by SLPs and others for their patients with swallowing dysfunction. They, rightly, want to know the bases for these recommendations and, finding that there are none, ask the reasonable question - why do it? They are more aware than GPs of the dangers of dehydration and malnutririon  in their patients as well as the hazards associated with enteral feeding and are disturbed by suggestions about "NPO" or "restricted fluids."  Without solid evidence to back it up, any recommendation must be carefully considered - and I don't mean only published research, of which there is really none with respect to much of the intervention proposed, but the underlying neurophysiology that would make sense of a given suggestion.
