[Dysphagia] NICU/peds protocol

We are trying to begin offering dysphagia services in our NICU/pediatric units but having some difficulty establishing a protocol for assessment. For those working with these populations, do you write reports as narrative or use some sort of assessment tool/form when performing evaluations? Have you developed your own protocols in your facilities? If anyone could forward sample eval protocol or form, would be much appreciated...
