ASHA has a paper on SLPs in the NICU which was completed last year. This
may be of some assistance for you. You can access it on the ASHA website.

We are trying to begin offering dysphagia services in our NICU/pediatric
units but having some difficulty establishing a protocol for assessment. For
those working with these populations, do you write reports as narrative or
use some sort of assessment tool/form when performing evaluations? Have you
developed your own protocols in your facilities? If anyone could forward
sample eval protocol or form, would be much appreciated...
