[Dysphagia] therapy
Agree with all that has been previously said, but bear in mind what Irene
Campbell-Taylor has taught (and many of us have seen): Aspiration (and
dysphagia) alone are NOT enough to cause medical problems. There must also
be bacteria in the oral cavity and immunosuppression. Many years ago I too
had a patient, at a city nursing home, who failed to follow any of my advice
despite awful MBS's. She ignored me, the facility ignored me, and the
patient never got sick (I followed her for years). Even when we better
understand the contributions of oral and pulmonic health to pneumonia, we
will not be (as one of my medical colleagues teaches) the "Medical Police".
If patients understand risks, that is give informed consent, they can choose
how to behave.
In your case, I'd confront the patient and just ask her. Is she intimidated
by the white coat and won't assert her true wishes? If she is ok with the
risk, document and d/c her.

Hi everyone,
I have one client iwht dysphagia who practices all the strategies in front
of me,a nd says yes she will do them, but as soon as I leave she does what
she wants. I have tried counceling sessions, educating the client and her
family, results of the MBS study, and nothign seems to convince her that she
needs to stay on ehr diet. Does anyone ahve any
