[Dysphagia] PICA



I had a fascinating client once with this very issue.  He was only 3 years 
old at the time and had only a mild developmental delay - pretty much your 
"typical looking" preschooler, but with some major feeding issues. 
Everything pointed to sensory based issues, with maybe some previous 
undiagnosed GERD as a suspect cause.  Any textures were a major issue.  Mom 
described PICA type behaviors such as picking the carpet piling and eating 
it and eating dirt and sand!  I was amazed given that he couldn't tolerate a 
grain of texture in his pureed food.  I watched him once in their playyard. 
He played nicely with his sib for a few minutes, then made a bee-line to the 
little plastic house and proceeded to run his fingers down every crease that 
held dirt and eat it.  He was cognitively aware enough to only do this when 
he didn't think I was watching.

I remembered at a conference I had attended with Kelly Dorfman, R.D. who 
suggested iron deficiency as a cause of PICA.  I urged Mom to get his labs 
done - his iron came out so low the doctor was amazed that he still had any 
energy to do anything.  He told Mom he would have to hospitalize his if the 
numbers didn't improve quickly with the supplements.  It took some time to 
get him regulated medically and his PICA did go away.  That was the first 
time I had seen that type of presentation in my 17 years of practice; it was 
a great learning experience for me!
