Sounds like recurrent aphthous ulcer (canker sore). These usually  begin as a 
round yellowish elevated spot surrounded by a red halo. Sometimes  these 
ulcers can be painful, particularly if they are irritated by movement or  from 
eating certain types of food. The loss of appetite when they occur would  seem to 
indicate some degree of pain. The cause is not known but can be  associated 
with GI disturbance as well as sensitivity to certain foods. There is  no 
treatment other than attempting to find out the cause in particular cases and  
eliminating it if possible.  It is a little unusual that ir recurs in the  same 
spot. If its appearance is not as described above I would suggest having it  
examined by the pediatrician.
