Regarding dysphagia and pernicious anemia.
There is nothing in the literature that I could find, but we had a case
this week of suspected R MCA stroke presenting with facial, tongue and
hand numbness.  On clinical assessment, she was found to have variable
laryngeal elevation.  Each bolus required multiple repeat swallows to
clear, and she had subtle throat clearing.  Unfortunately we were unable
to preform a MBS due to equipment breakdown.  I had requested a B12 and
RBC folate because she had a remote gastroplasty procedure.  She was
found to have significant B12 and Folate deficiency, and her symptoms
resolved after parenteral B12 and oral multi-vitamin supplementation. 
Her  DW-MRI was negative for stroke.  Diagnosis suggestive of b12
deficiency.  Interesting case!

I was also briefly looking into funding sources once in the past.  I
know Zygo has a little info on their website www.zygo-usa.com as does
Dynavox (and probably others), who I think has funding coordinators.
You may check into the medicaid info provided. If the pt happens to be
a
veteran, you can refer them to their local VA hospital in which case
it
MAY be paid for.

I am currently looking into a free trial with  Lingraphica for one of
my
aphasia patients.  The company will  determine whether any given
patient
has insurance coverage for  speech-generating devices, and if so, they
will loan it at no charge  for several weeks time for a trial first at
the clinic with the SLP,  and then at the patients' home.  I haven't
ever seen the device  before but I definitely want to give it a
try...no
harm done if it's  not a good match.  No cost to the clinic or the
patient.   I'll be glad to report back if/when we do go through the
free
trial...

I was wondering if anyone has come across dysphagia in pernicious
anaemia?  I have a patient who is Nil By Mouth and has a PEG.  She has
no other known diagnosis apart from Pernicious anaemia.  She has
difficulty managing her oral secretions, and is prone to chest
infections.  Her family report she has suffered from 'chronic
bronchitis' for as long as they can remember and has always had problems
with her oral secretions.

I use EndNote by Tomson [ http://www.endnote.com/ ]. It is available  
for both Windows and Macintosh operating systems.  I find it  
extremely versatile and have been very pleased with it.  The same  
company makes a higher-end product called ProCite [http:// 
www.procite.com ].  I used this in the past and would have continued  
with it, but the company has chosen not to upgrade the Mac version to 

work with OSX.  So, being a Mac user, I shifted to EndNote.  The  
program does have a field for "Notes".
