[Dysphagia] RE: Dysphagia Digest, Vol 30, Issue 8
At Royal Prince Alfred Hospital here in Sydney we are able to order modified
texture diets/fluids on the online diet ordering system and this refers to
all patients referred to Speech Pathology (ie NBM, oral intake). For special
diets e.g diabetic, we contact the Diet Aide or Dietitian. We do not need
medical approval to order any diets/fluids. With surgical patients, where
fluids/food are being introduced slowly, they may write diet/fluids as per
Speech Pathology in the medical notes if applicable.

Hi all,
At Auckland Hospital the SLTs advise the dietitians of the recommended
diet if it is not a straight normal one, and they organise the diet to
arrive on the ward with the hospital kitchen. We do not need the
approval of the medical team doctors to change a patients diet
consistency or amount, of course they can over ride our decisions if
indicated from a medical viewpoint.

I have worked at a few facilities that let RD change diet orders. In
terms of SLP writing orders, I have covered facilities that let us write
a TVO which the RN later calls in; or we could advance or reduce the
solid or liquid level of a diet if the pt was already ordered a diet.
We could not go from NPO to a diet or diet to NPO without MD order.
Where I am currently at many MDs write "diet per Speech".

Hello Colleagues!
The Continuing Education Committee of ASHA's Division 13 would like to
solicit your suggestions for continuing education topics. We would like
to
identify the areas where you and your colleagues feel you need
continuing education
support. For example, one issue that has been discussed is a "back to
basics" series that reviews anatomy and physiology of the swallow.
Another is
evidenced based support for therapeutic strategies. We are looking at
suggestions for topics to be covered in short courses/seminars at the
annual
convention, web forums, fact sheets etc. These materials would be
available to all
ASHA members and not just Division 13 members.
Please forward these to me privately. If the list is interested in the
topics collected, I will post.

I think this topic has been addressed on this list serve before but I
would
like to reviist it. I work in an acute care hospital setting. I am
often
faced with the problem of making a simple diet texture and or liquid
consistency adjustment by obtaining a telephone order from a difficult
to
reach physician. It has always been my understanding that this can only
be
done with a physician's order and physicians are the only ones allowed
to
write orders.That being said our dietary department recently wrote a
policy
stating that a registered dietician could adjust diet texture or write
orders for supplements. This was approved by the Pharmacy and
Therapeutics
Comittee and then by the Medical Review Board.
Orders are now written by dietician to adjust diet textures, po
supplements
etc. This is only after a diet has been intiated by the physician.
I would like to do something similiar to this which would allow the
Speech
Pathologists to write orders to change the texture of an existing diet
and
the viscosity level of liquids. This would be referenced to the
national
dysphagia diet standards.
Now for the question. Has any one had success with implementing this
type of
protocal? I realize we all have physicans that we work with regularly
which give us blanket telephone orders priviledges,but I would like to
make
this avaliable as a protocal which pertains specifically to the SLP
staff to
make this change independently.
