For those of you who work in an acute care setting (where patient length of
stay can be very short sometimes), do you complete a discharge summary when
a patient discharges from the acute care setting?
Would it depend on where the patient discharges to?
Is there anything in our national practice code which says that discharge
summaries need to be completed when a patient leaves acute care? Would the
last treatment note suffice?
