[Dysphagia] Frazier Water protocol screening form

I work at a residential facility for people with DD.  We are currently
in the process of proposing to implement the Frazier Free Water
Protocol.  Does anyone who implements this protocol have a
screening/assessment form that they use to document findings and for
their record?  If so, could you please share the form with me.
