[Dysphagia] Logemnn Robbins study



Hi all
Despite all the to-ings and fro-ings that have been going on since
Irene's initial point, we agree that dysphagia management of the elderly
requires considerable review, that there still isn't sufficient evidence
for or against thickened fluids. We have been conducting a study using a
Water Protocol with mostly elderly patients with various medical
diagnoses and co-morbidities, using a Single Subject (Experimental)
Research Design with multiple replications. This is one attempt to
commence amassing evidence also, for "safety" of at least supplementing
with water. So far with positive results. We are presenting a paper
describing the results at the upcoming Speech Pathology Australia
Conference in Sydney, at the end of this month. We have also submitted a
literature review/discussion paper for publication as a Forum.

I wonder how individuals are altering their management practices in
light of the recent revelation of the dehydrating effects of thickeners
and the failure of thickening and chin tuck to prevent aspiration. The
authors acknowledge that the ciliary clearance of aspirated thickened
fluids probably causes the resultant pneumonia.  This is undoubtedly
more true of gum based thickeners. It would seem that management,
especially of the elderly, requires immediate alteration.
